Formal processes, data, technologies and specialised teams each hold only part of the picture. People bridge the gaps by interpreting context, reconciling conflicting information, carrying knowledge between boundaries and exercising judgement when rules do not fit reality.

This human integration makes organisations work, but relying on it invisibly creates risk. When a few people continually compensate for fragmented systems, they become overloaded, knowledge becomes trapped and their departure exposes connections the organisation never formally understood.
